Participation by 75% of eligible employees is required for all groups
enrolling through the Las Vegas Chamber of Commerce Group
Health Benefits Program. This does not include those employees with
other coverage.
Waiver forms must be completed and signed by all
employees not enrolling for coverage. If the employee waives due to
other coverage, they need to provide details of the other coverage
and or a copy of the member identification card for the other coverage. In all cases, the employer must contribute at least 50% of
the employee-only premium for medical, dental, vision and 100%
for Life/AD&D.
