Q. What are the requirements for the Las Vegas Chamber of Commerce group health insurance program?
A.
Your business must be a partnership or a corporation or a subsidiary or an associated firm, provided such firm has authority to purchase group health coverage for the employees of the applicant group. In some instances an employer or corporation may control several subsidiary companies whose employees may be covered under the parent company’s group policy.
Q. Can a sole-proprietor qualify?
A.
Your business must have two or more full-time employees and there must be a legitimate employee/employer relationship within the group. A single billing address must be provided. A group is ineligible if it was formed solely for the purpose of obtaining insurance. A photocopy of a business license must be provided.
Q. Our company currently has a policy with Health Plan of Nevada. Can we switch to the Chamber's group insurance plan?
A.
If a group currently has coverage with Health Plan of Nevada or Sierra Health and Life Insurance Company, Inc. they may move to a Chamber plan on their anniversary date or annual renewal date as long as there is no lapse in coverage. These groups must reapply for coverage. However, any group previously insured by Health Plan of Nevada or Sierra Health and Life whose coverage is terminated may not apply for new coverage within 12 months of the termination date.
If a prospective Chamber group makes application for coverage and declines to accept their final Medical Underwriting Rating Factor (MURF) as assigned by Medical Underwriting, Health Plan of Nevada will not consider reapplication as a new Chamber group within 90 days of the group’s initial effective date.
Q. Can any insurance agent enroll my company?
A. If you are utilizing the services of a broker, the broker must be a current member of the Las Vegas Chamber of Commerce and furnish documentation to that effect.
All groups and employees must be located within the Health Plan of Nevada Southern Nevada area in order to be eligible for the Las Vegas Chamber of Commerce Group Health Benefits Program.
Q. Are the part-time employees at my company able to participate in this program? What about spouses and dependants?
A.
Eligible employees include all active employees who work at least 30 hours per week. Coverage is also available to dependent spouses and children up to age 19, full-time students to age 24 and handicapped children. Q. What if some of my employees do not wish to participate in the program?
A.
Participation by 75 percent of eligible employees is required for all groups enrolling through the program. This does not include those employees with other coverage. Waiver forms must be completed and signed by all employees not enrolling for coverage. In all cases, the employer must contribute at least 50 percent of the employee-only premium for medical, dental, vision and 100 percent for life/accidental death/dismemberment.
